Medical Records. Authorized parents or legal guardians have the right to review or receive a copy of their child's medical records. Obtaining a Copy of a Medical Record. Please print, complete and sign the request form below to obtain copies of your child’s medical records. Mail or fax your signed request form to the address or fax number located at the bottom of the authorization form. You may also email the form to roi@childrenscolorado.org or leave the form with the Release of Information department during business hours. Please note: Release of Information cannot review, discuss or interpret medical records. Please direct these questions to your healthcare provider. Receiving Your Requested Records. Processing medical record requests will take 7 to 10 business days from date of receipt. Records will be released and delivered in the method indicated on the authorization form. Occasionally delays occur and we will attempt to contact you if extra time is needed to process your request. Please bring a photo ID if you choose to pick up the medical records in person. Payment for Records. Colorado State Law authorizes Children’s Hospital Colorado to charge for copying medical records. The charges are due at the time you receive your records. Release of Information accepts check and credit card payments only. If you have a medical record invoice, you can pay online. If you have questions related to the payment of your medical records, please contact the Release of Information department. Please direct any questions related to your hospital bill to Patient Financial Services 720-777-6422. Advice When Requesting Copies of Medical Records. Requests for all information (including progress notes, consent forms, flow sheets, etc.) can delay processing, become very costly and can sometimes be much more information than you really need.
